Our History

Central Minnesota Health Network (CMHN) was founded in 2015 with the goal of developing a clinically integrated network of physicians and hospitals that collaborates on patient-centric initiatives to achieve the goals of improving the quality of care, reducing costs and improving patient experience to foster improvement in the overall health of the communities within central Minnesota.

Key CMHN Objectives:

• Foster greater physician collaboration
• Promote better care for the central Minnesota population
• Foster cultural change
• Lower the "cost of care" price point
• Prepare for value-based payment models
• Support independent physicians who desire alignment
• Leverage investments in enterprise data warehouse (EDW)
• Expand infrastructure capabilities to support population health management
• Demonstrate improved health outcomes for patients
